Prior to your appointment: -After providing the requestor's name, contact number and email, you will be emailed a portal link. -You will complete all required information about all involved parties and submit copy of Request for Evidence. -Once validated, pricing will be calculated. After payment is submitted and processed, then our office will contact you to schedule an appointment. What you will need to bring the day of your appointment: - An UNEXPIRED government-issued ID (DL, State ID, Military ID or US Passport) for all parties 18 yrs of age or older. Identification such as employee badge, school/college/university ID are not acceptable. -You will be photographed for testing purposes. Refusal of photography nullifies testing and therefore WILL NOT be performed.
