This is for a court-ordered paternity test. What you will need to bring the day of your appointment: - The original court order request form (not a photocopy) - A government issued ID for all parties 18 yrs of age or older. (If no ID is present, you will be photographed for testing purposes. Refusal of photography nullifies test.) Note: If paying separately, list in comments the name of original tester, so appointments will be linked.
