Service Description

What you will need to bring the day of your appointment: - The original court order request form (not a photocopy) - An UNEXPIRED government-issued ID (DL, State ID, Military ID or US Passport) for all parties 18 yrs of age or older. Identification such as employee badge, school/college/university ID are not acceptable. -You will be photographed for testing purposes. Refusal of photography nullifies testing and therefore WILL NOT be performed. -A legal guardian of the child in question, who can provide consent, if the child is under the age of 18.
