This is for a court-ordered prenatal test. What you will need to bring the day of your appointment: - The original court order request form (not a photocopy) - A government issued ID for all parties 18 yrs of age or older. (If no ID is present, you will be photographed for testing purposes. Refusal of photography nullifies test.) What to expect: -Blood draw -Signatory documents -
